مزایای تشخیص سریع مننژیت باکتریایی به روش مولکولی در مقایسه با کشت و مشاهده مستقیم میکروسکوپی مایع نخاع

<b>زمینه و اهداف:</b> مننژیت حاد باکتریال یک عامل مهم مرگ و میر باقی مانده که در افراد نجات یافته، ممکن است با ایجاد ضایعات عصبی دایمی همراه گردد. لذا، تشخیص سریع اتیولوژی مننژیت باکتریال و درمان مناسب آن یک امر حیاتی می باشد. از این رو، هدف این تحقیق مقایسه روش مولکولی MultiplexPCR با روش های باکتریولوژیک و نیز مشاهده مستقیم مایع نخاع جهت تشخیص سریع باکتری های شایع عامل مننژیت می باشد.
<br><b>روش بررسی:</b> در این تحقیق، 150 نمونه مایع نخاع بیماران مشکوک به مننژیت را با استفاده از PCR هدف مند شده با پرایمرهای عمومی بر اساس ژن 16S rRNA و نیز پرایمرهای اختصاصی جهت باکتری های <i>نیسریا مننژیتیدیس</i>، <i>استرپتوکوکوس پنومونیه</i> و <i>هموفیلوس اینفلونزا</i> استفاده گردید. همچنین، سایر روش های باکتریولوژیک و نیز میکروسکوپی مورد بررسی قرار گرفت.
<br><b>یافته ها:</b> استفاده از چند روش تشخیصی همزمان، امکان تشخیص سریع اتیولوژی باکتریال را در افراد مبتلا به مننژیت مهیا نمود. چنانچه با روش مولکولی تشخیص اختصاصی از 150 نمونه مایع نخاع 9 مورد نیسریا مننژیتیدیس تایید گردید. در حالی که کشت باکتریولوژیک تنها در 6 مورد نیسریا مننژیتیدیس را نشان داد ولی مشاهده لام مرطوب وجود 8 مورد مننگوکک را تایید نمود.
<br><b>نتیجه گیری: </b>نتایج این تحقیق نشان داد، روش های مولکولی PCR یگانه و چندگانه برای تشخیص عامل مننژیت باکتریایی از کشت باکتریولوژیک و نیز مشاهده مستقیم از حساسیت بیشتری برخوردار است. با این حال، قضاوت بر اساس مشاهده میکروسکوپی هر چند کیفی است. ولی مشاهده مستقیم مرفولوژی باکتری و نیز سایر شواهد در نمونه مایع نخاع علاوه بر راهنمایی جهت طراحی پروتکل مناسب تشخیص قطعی و امکان انتخاب آنتی بیوتیک مناسب را جهت درمان نیز فراهم می نماید.

<b>Background and Objectives:</b> Acute bacterial meningitis has remained an important cause of death and
neurological damages among survivors. Rapid diagnosis of bacterial meningitis is crucial for the early
targeting of antimicrobial therapy. The aim of this study was to develop and appply a PCR assay for rapid
diagnosis of meningitidis and to compare the results with those obtained by conventional bacteriology.
<br><b>Material and methods:</b> We assessed 150 cerebrospinal fluid (CSF) specimens from suspected patients by
PCR targeting 16S rRNAgene with specefic primers for <i>Neisseria meningitidis,</i> <i>Sterptococcus pneumonia</i>
and <i>Heamophilus influenza</i>. All speciemns were also examined by conventional bacteriology. <br>
<b>Results:</b>The rapidity of diagnosis increased when bacteriological methods were combined with PCR. Of
150 speciemens tested, 10 were positive for <i>Neisseria meningitidis</i> in PCR. Direct microscopy and bacterial
culture found 5 and 8 cases infected with this organism respectively.
<br><b>Conclusion:</b> PCR was more sensitive than direct microscopy and culture for detection of <i>Neisseria
meningitidis</i>. However, direct microscopy may provide evidences for the quality of specimens and presence
of other organisms in the samples. Wet- mount direct microscopy showed morphology and arrangements of
the observed organisms that may be helpful in presumptive identification of certain bacteria such as gram
negative bacilli and cocci.. Moreover, the observed organisms may be useful in correct selection of culture
media in the laboratory and prescription of appropriate therapy by physicians in a quickest time.
