en جداسازی، شناسایی و مقاومت آنتی بیوتیکی باکتری های بی هوازی در خلط بیماران فیبروز سیستیک از مرکز طبی کودکان تهران، ایران Isolation, Identification and Antibiotic Resistance of Anaerobic Bacteria in Sputum of Cystic Fibrosis Patients from Children's Medical Center of Tehran, Iran باکتری شناسی پزشکی Medical Bacteriology مقاله پژوهشی Original Research Article زمینه و اهداف:  به دنبال نقص ژنتیکی در فیبروز سیستیک (CF)، تجمع موکوس و شیب هیپوکسیک در ریه ها ایجاد می شود که شرایط را برای استقرار باکتری های بی هوازی ایجاد می کند. این مطالعه به بررسی تفاوت بین شیوع برخی از باکتری های بی هوازی و مقاومت آنتی بیوتیکی در گروه های سالم و بیماران CF می پردازد. مواد و روش کار:  در این پژوهش، باکتری های بی‌هوازی در نمونه های خلط بیماران CF (۵۰ نفر) و افراد سالم (۱۸ نفر) در شهر تهران به روش کشت در سال ۱۴۰۱ مورد بررسی قرار گرفتند. نمونه ها روی بروسلا بلاد آگار (BA)، BHI آگار با ونکومایسین،LKV  و BA با نئومایسین و ونکومایسین کشت داده شدند. جدایه های گرم منفی با روشهای بیوشیمیایی و جدایه های  گرم مثبت با استفاده از توالییابی ۱۶s rRNA شناسایی شدند.MIC  جدایه های باکتریایی برای آمپی سیلین، کلیندامایسین و آمپی سیلین/سولباکتام با استفاده از نوارهای  E-test  بررسی شد. یافته ها:  باکتری های بی هوازی به ترتیب در ۳۳ درصد و ۲۴ درصد از گروههای سالم و بیماران CF جداسازی شد. جدایه های شناسایی شده عبارتند از :Dialister spp. ،Fusobacterium spp. ، Lactobacillus spp.  و Prevotellaspp.  . جدایه های Fusobacterium  از گروه سالم به آمپی سیلین حساس بودند و ۳۳ درصد از جدایه های Fusobacterium از گروه CF نسبت به این آنتی بیوتیک مقاومت نشان دادند. نتیجه‌گیری:  در این مطالعه وجود باکتری های بی هوازی در ریه افراد سالم و CF شناسایی شد. فراوانی کل این باکتری ها در گروه سالم بیشتر از گروه CF می باشد. فراوانی باکتری گرم مثبت Lactobacillus، به عنوان باکتری بالقوه مفید، در بیماران CF کمتر از افراد سالم است. Background and Aim: Following genetic defects in cystic fibrosis (CF), mucus accumulation and hypoxic gradients develop in the lungs creating conditions for the anaerobic bacterial colonization. This study investigated the difference between the prevalence of some anaerobic bacteria and antibiotic resistance among healthy and CF groups. Materials and Methods: In this research, anaerobic bacteria were investigated in sputum samples of the CF patients (50 people) and the healthy individuals (18 people) in Tehran, Iran by culture method during 2021-2022. The sputum samples were cultured on Brucella blood agar, Brain heart infusion agar with vancomycin, Laked brucella blood agar with kanamycin and vancomycin, and Brucella blood agar with neomycin and vancomycin. Gram-negative and Gram-positive isolates were identified using biochemical methods and 16S rRNA sequencing, respectively. In addition, the minimum inhibitory concentration (MIC) of the bacterial isolates was tested for ampicillin, clindamycin and ampicillin/sulbactam using E-test strips. Results: In the culture-based method, anaerobic bacteria were isolated in 33% and 24% of the healthy and CF groups, respectively. Bacterial isolates detected include Dialister spp., Fusobacterium spp., Lactobacillus spp., and Prevotella spp. The Fusobacterium isolates from the healthy group were sensitive to ampicillin, but 33% of Fusobacterium isolates from the CF group showed resistance to this antibiotic. Conclusion: In this study, the presence of anaerobic bacteria in the lungs of healthy and CF individuals was determined. The total frequency of these bacteria was higher in the healthy group than the CF group.
