fa کواگولوپاتی در بیماران مبتلا کووید 19 و درمان اندوواسکولار، گزارش 4 مورد ویروس شناسی پزشکی Medical Virology مقاله گزارش مورد Case report Article <div style="text-align: justify;"><span style="font-size:14px;"><span style="font-family:IRANsans;">پاندمی <span dir="LTR">COVID-19</span> که به وسیله عفونت تنفسی <span dir="LTR">SARS-CoV-2</span> ایجاد شده است، تبدیل به یک تهدید جهانی شده است. بروز عوارض ترومبوتیک در بیماران مبتلا به <span dir="LTR">COVID-19</span> بالا می‌باشد و در مطالعات متعددی به آنها پرداخته شده است. &nbsp;در این میان کمتر به عوارض هموراژیک بیماران مبتلا به کووید 19 توجه شده است. لذا در این مقاله به ارائه 4 مورد بیمار مبتلا به کووید 19 با عوارض همورژایک پرداخته شده است.<br> هر چهار بیمار در بخش مراقبت‌های ویژه بستری و در حال دریافت درمان‌های روتین کووید 19 از جمله هپارین پروفیلاکسی بودند. در سیر بستری هر 4 بیمار دچار هماتوم شیت رکتوس و خونریزی رتروپریتوئن شدند و جهت کنترل خونریزی تحت آمبولیزاسیون قرار گرفتند.<br> آمبولیزاسیون شریان اپی گاستریک تحتانی و تنه قدامی شریان ایلیاک داخلی تحت هدایت فلوروسکوپی و آنژیوگرافی انجام شد. خونریزی فعال بیماران کنترل گردید. سه بیمار پس از سیر درمان مناسب از بیمارستان ترخیص شدند. یک بیمار با توجه به شدت درگیری ریوی ناشی از کووید 19 با توجه به سن بالا و حجم حونریزی متاسفانه فوت شد.<span dir="LTR"><span style="font-size:12.0pt;"></span></span><br> در ابتدا بروز خونریزی در این بیماران به عوارض هموراژیک ناشی از هپارین نسبت داده شد. اما حجم خونریزی رخ داده در بیماران و علایم با عوارض ناشی از دوز پروفیلاکسی هپارین چندان قابل توجیه نبود، لذا به تدریج توجه به کواگولوپاتی در بیماران مبتلا به کرونا معطوف شد. مطالعات بیشتری نیاز است تا نقش عوامل ایجاد کننده و عوارض ناشی از کوآگولوپاتی را در بیماران مبتلا به <span dir="LTR">COVID-19</span> و درمان مناسب آن ها روشن سازد.</span></span></div> <div style="text-align: justify;">COVID-19 pandemic due to the respiratory infection of SARS-CoV-2 has now become a global threat. The thrombotic complications are common in COVID-19 patients which have been extensively addressed in various studies. The hemorrhagic complications, however, have received less attention. In this article, 4 cases of COVID-19 patients with hemorrhagic complications are presented.<br> All 4 patients were hospitalized in ICU and received routine COVID-19 treatments such as heparin prophylaxis. During hospitalization, all four patients had rectus sheath hematoma and retroperitoneal hemorrhage. They underwent embolization to control hemorrhage.<br> Embolization of the inferior epigastric artery and the anterior trunk of internal iliac artery was carried out under fluoroscopy-guided angiography. The active hemorrhage of the patients was controlled. Three patients recovered after the treatment and were discharged while one patient, unfortunately, died due to the severity of the pulmonary involvement, old age, and hemorrhage.<br> First, the hemorrhage of these patients was assigned to the hemorrhagic complications due to the use of heparin. But the hemorrhage volume was not justifiable with the symptoms and complications of prophylaxis dosage of heparin.
