زمینه و اهداف:  عفونت های بیمارستانی ناشی از باکتری های مقاوم به آنتی بیوتیک و همچنین گسترش سریع آن ها به عنوان یکی از تهدیدات سلامت عمومی محسوب می شود. هدف این مطالعه تعیین فراوانی ژن‌های کدکننده بتالاکتاماز طیف گسترده  (ESBL)در باکتری‌های گرم منفی بود.
مواد و روش کار:  شناسایی و الگوی حساسیت آنتی بیوتیکی ۱۶۵ ایزوله به ترتیب با روش بیوشیمیایی و دیسک دیفیوژن انجام شد. علاوه بر این، غربالگری و تایید حضور ژن‌های ESBL به روش تست ترکیبی دوتایی (DDCT) انجام شد. وجود ژن های کدکننده ESBL در هر ایزوله با روش واکنش زنجیره ای پلیمراز  (PCR) شناسایی شد.
یافته ها:  از ۱۶۵ ایزوله، ۸۳ سویه به تمام آنتی بیوتیک ها مقاوم بودند. کمترین فراوانی مقاومت در مقابل جنتامایسین و همچنین بیشترین فراوانی مقاومت در سفوتاکسیم و سفازولین مشاهده شد. از بین تمامی سویه های مورد تجزیه و تحلیل، ۵۰ (۳۰/۳۰%) و ۸۰ (۴۸/۴۸%) ایزوله به ترتیب از نظر فنوتیپی و ژنوتیپی ESBL مثبت بودند. شایع ترین ژن های کد کننده ESBL به ترتیب SHVOS و SHV-۱ با فراوانی ۳۴ (۶۱/۲۰ %) و ۳۶ (۸۲/۲۱%) بودند.
نتیجه‌گیری:  فراوانی تولید ESBL و شیوع بالای ژن‌های blaSHV و blaCTX-M در کلبسیلا پنومونیه و اشریشیا کلی در ایزوله‌های باکتریایی ذکر شده در کرمانشاه رو به افزایش است. اما برخلاف برخی گزارش‌های قبلی از کرمانشاه، شیوع ژن‌های کدکننده ESBL در سودوموناس آئروژینوزا کم بود و ژن blaVEB یافت نشد.
