بررسی اپیدمیولوژیک انتروکوک مقاوم به ونکومایسین در کودکان مبتلا به لوسمی لنفوبلاستیک حاد بستری در بیمارستان های حضرت علی اصغر (ع) و محک تهران در سال 87-1386

<b>زمینه و اهداف:</b> انتروکوک های مقاوم به ونکومایسین (Vancomycin resistant enterococci=VRE) ، در بیماران مبتلا به نقص ایمنی عامل عمدهه ابتلا و مرگ و میر هستند. هدف از این مطالعه تعیین فراوانی حاملین انتروکوک مقاوم به وانکومایسین در کودکان مبتلا به لوسمی لنفوبلاستیک حاد، تعیین اپیدمیولوژی مولکولی آن و عوامل خطرساز موثر در شیوع آن در سال 87-1386 بود. <br>
<b>روش بررسی:</b> در یک مطالعه توصیفی مقطعی نمونه مدفوع 130 کودک مبتلا به لوسمی لنفوبلاستیک حاد مراجعه کننده به 2 بیمارستان تهران ( علی اصغر و محک) بر روی محیط های انتروکوک آگار و آگار خوندار کشت داده شد. ارگانیسم های جدا شده به روش باکترویولوژی نعیین هویت شدند. حداقل غلظت بازدارنده وانکومایسین به روش macrodilution تعیین شد. سرانجام PCR با پرایمر های VanAو VanB انجام گردید. سایر اطلاعات از طریق پرسشنامه و با رجوع به پرونده بیماران تکمیل شد. برای تجزیه و تحلیل داده ها از آزمون های آماری STT ،Mann whithney new، Chi-sqare، Fisher exactو Anova و Kruskal-Wallis استفاده شد.<br><b> یافته ها:</b> از کشت مدفوع 130 کودک ( با متوسط سن 6.5 سال) در 95 نمونه (73 %) انتروکوک جدا شد. از 23 سویه ( 24.7 %) انتروکوک مقاوم به وانکومایسین، 26 سویه (78.8 %) واجد فنوتیپ VanA و 7 سویه ( 21.2 %) دارای فنوتیپ VanB بودند. سابقه بستری در ICU با اکتساب انتروکوک ارتباط معنی داری داشت. (P=0.03) و با اکتساب VRE هم روندی به سمت معنی دار شدن نشان داد. (P=0.07) بین وجود VanA و VanB در سویه های VRE و متغییر سن تفاوت معنی دار مشاهده شد. (P=0.07). بین متغییرهای جنس، مدت زمان بیماریف تعداد بستری های قبلی، وجود بیماری همزمان دیگر، سابقه نوتروپنی شدید در یک ماه قبل از جمع آوری نمونه، سابقه مصرف آنتی بیوتیک در 3 ماه قبل و اکتساب VRE ارتباط معنی دار مشاهده نشد. (P>0.05)
<br><b>نتیجه گیری:</b> مطالعه حاضر نشان می دهد که شیوع VRE در کودکان ایراتی مبتلا به لوسمی لنفوبلاستیک حاد زیاد است. اجرای برنامه های مداخله ای جهت کنترل آن بسیار ضروری به نظر می رسد.
<b>Bachground and Objectives:</b> vancomycin resistant Enterococcus (VRE) in immune deficient patient is a significant factor for increase in morbidity and mortality. The aim of this study was to determine VRE carriage rate , its molecular epidemiology and to identify risk factors for acquisition of VRE in children with Acute lymphoblastic leukemia (ALL) in 2007-2008.
<br><b>Material &Methods:</b> In a cross-sectional study, stool samples of children with ALL in Aliasghar and Mahak hospitals were randomly analyzed for detection of VRE . Enterococci were identified by conventional bacteriologic methods and vancomycin resistance was detected by disk diffusion method and E-test. MIC of vancomycin wase determined by macrodilution method. VanA and VanB genes characterization was performed by a multiplex polymerase chain reaction (PCR), which also confirmed the identification. Questionnaires of patient was done to determine risk factors. Data were analyzed by STT , Mann whithney new , Chi-sqare , Fisher exact , Anova and Kruskal-Wallis tests.
<br><b>Results:</b> In the present study, of 130 child with ALL admitted to our hematology/oncology an infection units, 33(34.7%) of the 130 patient were colonized with VRE(25.3% of total cases) and remainder of patients were colonized with vancomycin-sensitive strains (VSE), serving as controls. 26 isolates(78.8%) of VRE detected with VanA phenotype and seven isolates(21.2%) with VanB phenotype. In the univariate analysis history of ICU admission(P=0.03)was significantly associated with Enterococcus colonization and had a trend with VRE colonization. age (P=0,007)was significantly associated with VRE phenotype .There was no significant relationship between gender, duration of disease ,number of admission , another disease, history of severe neutropenia among one month ago and history of antibiotic use in three month ago and VRE colonization.
<br><b>Conclusion:</b> VRE prevalence is high in children with ALL in Iran ,indicating a prompt intervention strategy in the control of VRE particularly in PICU.
