زمینه و اهداف: کلبسیلا پنومونیه به دلیل ایجاد طیف وسیعی از بیماری‌ها و مقاومت آنتی‌بیوتیکی بسیار موردتوجه است. مقاومت به فلوروکوئینولون‌­­ها در این باکتری­های گرم منفی، ممکن است درنتیجه موتاسیون کروموزومی در ژن‌های gyr و par باشد. هدف از این مطالعه بررسی الگوی مقاومت به فلوروکوئینولون­‌ها و ارتباط آن با موتاسیون در ژن parC در میان جدایه‌های بالینی کلبسیلا پنومونیه هست.
روش کار: در این مطالعه توصیفی- تحلیلی، تعداد ۹۵ کلبسیلا پنومونیه پس از تشخیص بیوشیمیایی و مولکولی، ازنظر مقاومت به آنتی­بیوتیک­های مختلف، مطابق توصیه موسسه استاندارد آزمایشگاهی و بالینی CLSI بررسی و جدایه­‌های مقاوم به سیپروفلوکساسین غربالگری شدند. برای بررسی وجود موتاسیون در ژن parC در کلبسیلا پنومونیههای مقاوم به سیپروفلوکساسین، از طریق تکثیر این ژن به روش PCR و سپس تعیین توالی قطعه تکثیرشده و درنهایت مقایسه آن با توالی ژنوم باکتری استاندارد موجود در سایت NCBI، از طریق نرم‌افزارهای آنلاین، Insilico و Clustalw۲، بلاست گردید.
یافته‌ها:نتایج تست آنتی­بیوگرام حاکی از گسترش باکتری­های Multi Drug Resistant (MDR) بود. ۳/۱% جدایه­ها به همه­ی ۱۳ آنتی‌بیوتیک موردمطالعه و ۲/۲۴% جدایه‌­ها سیپروفلوکساسین مقاوم بودند. بیشترین و کم‌ترین درصد مقاومت آنتی‌بیوتیکی به ترتیب برای آمپی‌سیلین و آموکسی­سیلین (۹۸/۹%) و ایمی­پنم (۱۳/۶%) تعیین شد. آنالیز توالی یابی ژن parC نشان داد از ۲۳ جدایه مقاوم به سیپروفلوکساسین، ۱۶ جدایه در کدون ۸۰ (I۸۰S)، یکی در کدون ۸۴ (E۸۴K) و ۳ جدایه نیز دچار جهش در چارچوب بودند.
نتیجه گیری: جهش‌ در ژن parC می‌­تواند یکی از عوامل اصلی مقاومت به فلوروکوئینولون‌­ها و افزایش باکتری­‌های MDR و ایجاد عفونتهای بیمارستانی باشد.
